Indications:

This formula is for the treatment of chronic low back pain due to a combination of kidney vacuity (yin or yang) plus blood stasis and possible lingering wind damp evils. It may also be used to treat postmenopausal osteoporosis.
 

The signs & symptoms of kidney vacuity include:

  • Low back and knee soreness and limpness

The signs & symptoms of blood stasis include:

  • Fixed, localized pain

  • A dark, sooty facial complexion

  • Purple lips and nails

The signs & symptoms of wind damp impediment include:

  • Joint pain which is worse on exposure to cold and dampness

The signs and symptoms of liver depression include:

  • lower abdominal distention/cramping

  • irritability

  • rib-side ain

  • premenstrual breast distention and pain

  • a bowstring pulse

  • chest oppression

Combination:
If there is predominant yin vacuity with vacuity heat signs and symptoms, this formula may be combined with either Zhi Bai Di Huang Wan (Anemarrhena & Phellodendron Rehmannia Pills) or Da Bu Yin Wan (Greatly Supplement Yin Pills). If there is predominant yang vacuity, it may be combined with Shen Qi Wan (Kidney Qi Pills).

 

Formula explanation:
This formula is based on Dr. Zhang’s theory of kidney vacuity and blood stasis. According to Dr. Zhang, if there is kidney vacuity, there must be blood stasis. It is well known within Chinese medicine that the kidneys govern the bones and marrow of the entire body and that the low back is the mansion of the kidneys. When the kidneys become vacuous and weak, the low back typically becomes sore. This is why low back pain or soreness is one of the pathognomonic symptoms of kidney vacuity. However, it is also said that, “[In] enduring disease, there must be stasis.” In this case, kidney vacuity is the root and blood stasis is the tip or branch; kidney vacuity is the cause and blood stasis is the fruit. It is also possible for there to be lingering wind damp evils lodged in the low back, thus causing impediment to the flow of qi and blood in that region. In that case, wind damp evils have taken advantage of vacuity to enter and lodge, obstructing and blocking the free flow of qi and blood. While a number of other formulas may treat low back pain by supplementing the kidneys and quickening the blood, these other formulas typically do not take into account that it is spleen vacuity which leads to kidney vacuity with age. When this occurs, the spleen vacuity does not suddenly disappear. Rather, there is then a dual spleen-kidney vacuity (i.e., spleen qi and kidney yin vacuity, spleen qi and kidney yang vacuity, or spleen qi and kidney yin and yang vacuity). This formula takes this spleen vacuity into account, thus promoting the interaction and mutual support of the former and latter heavens.

 

Within this formula, Shu Di, Gou Qi Zi, and Niu Xi supplement liver blood and enrich kidney yin, while Tu Si Zi, Du Zhong, Xu Duan, Bu Gu Zhi, Gu Sui Bu, and Ba Ji Tian all supplement the kidneys and invigorate yang. However, even all these yang supplements nourish liver blood. Therefore, although they supplement yang, their action is relatively balanced. In addition, Niu Xi, Du Zhong, Xu Duan, and Ba Ji Tian are famous for their ability to strengthen the low back and alleviate low back pain. Tu Si Zi and Bu Gu Zhi also supplement the spleen, the latter heaven root. When combined with Ren Shen, which supplements the qi of all five viscera including the kidneys, they address the underlying spleen vacuity that typically precedes and leads to kidney vacuity with age. Dang Gui supplements the blood and nourishes the liver. It also quickens the blood and transforms stasis. Chi Shao and Chuan Xiong quicken the blood, dispel stasis, and stop pain. Wu Jia Pi is a wind dampness treating medicinal. This means that it treats impediment pain. It is particularly indicated when there is a combination of wind damp impediment with liver-kidney vacuity. It is often combined with Du Zhong, Xu Duan, and Niu Xi for the treatment of low back pain in older patients.
 

Dosage:
